- Split adapter inventory into Paper (Phase 3, tiny-aya-base/, the-stack-v2-dedup) and Preliminary (Phase 2, flat folders, the-stack v1) sections
- Fix broken usage paths: paper adapters load from the tiny-aya-base/ subfolder
- Add Provenance & Manifest section + MANIFEST.md (path -> phase -> corpus map)
- Note standalone v1 repos renamed to phase-2-the-stack-v1-* and deprecated here
